This form is used to request accounts for the following: Network Accounts (UHMC), SMS/UHIS, RAS, Eclipsys, Lawson, Radiology IDX, Radiology PACS, Muse and Cerner (PowerChart, SurgiNet, EWS or PharmNet)

Users will now receive a Temporary Access Pass (TAP) to setup their accounts. The process for that can be found here.
