Please indicate your Name (Credentials/School , Volunteer/Pay) on the sign-up link
e.g: John Smith (SBUH RN, P) or Joan Smith (SON4, V)
 

Do not remove any names on the scheduling link or edit any numerical lists 
Do NOT sign up on more than one link 

Volunteers are expected to arrive 30/60 minutes prior to the start of their shift for training

 

Medical Consult
  • Only for those who are eligible to be medical consultants
  • Including:  - MDs/DOs/NPs/PAs only

Vaccinator
  • Only for those who are eligible to vaccinate
  • Including:
    • PAs, NPs, RNs, LPNs
    • 3rd/4th year medical student
    • 4th year dental student
    • nursing students* (if signed off by Coordinator)

Data/Registration/Screener
  • Only for clinical personnel who are not eligible to vaccinate
  • Including:
    • 2nd year medical student
    • 2nd/3rd year dental student
    • PA students
    • any other professional students
    • nursing students* (who are not yet signed off)
    • Roles include: medical screener, data/registration
