Please indicate your Name (Credentials/School , Volunteer/Pay) on the sign-up link
e.g: John Smith (SBUH RN, P) or Joan Smith (SON4, V)
 

Do not remove any names on the scheduling link or edit any numerical lists 
Do NOT sign up on more than one link 

Volunteers are expected to arrive 30/60 minutes prior to the start of their shift for training

 

Data/Registration
  • Only for clinical personnel who are not eligible to vaccinate
  • Including:
    • 2nd year medical student
    • 2nd/3rd year dental student
    • PA students
    • nursing students* (not signed off to vaccinate)
    • any other professional students
  • Roles include: medical screener, data/registration

  • Including:
    • undergraduate students
    • professional students
    • other non-clinical personnel
  • Roles include: flow control, eligibility, data/registration if needed

Vaccinator
  • Only for those who are eligible to vaccinate
  • Including:
    • PAs, NPs, RNs, LPNs
    • 3rd/4th year medical student
    • 4th year dental student
    • nursing students* (can NOT sign up if they haven’t been signed-off by their instructor to vaccinate alone )
  • You MUST have already sent in your NYS DOH Vaccinator Course Certifications to to your coordinator
