Please complete ALL of following (5) forms prior to working first shift at Vaccine POD


Volunteers are expected to arrive 30/60 minutes prior to the start of their shift for training

NOTE: If you are current employee and in the system an I-9 is not needed as that is for students to be paid.  The BLS and HIPPA Privacy Certificates should go to the Vaccine Pod Staffing email address: Vaccine Pod Staffing Vaccine_Pod_Staffing@stonybrookmedicine.edu  in a PDF format.
